Please review the following before submitting your interest form.

• A sitting fee will be collected upon check in, to cover the cost of the facility and providers. This is nominal in comparison to the standard retail price of these services. The fee will be communicated to you prior to arriving. It ranges from $150- $500 depending on the training.

• Model must have a flexible schedule the day of training. Please allow for the time of your training slot to be upto 3 hours.

• Model must not have had any vaccines, or dental work, 2 weeks prior to training date. Model does not need to be a Synergy client, however, model does need to disclose previous treatments/procedures that may impact their results.

• Model must be comfortable with multiple attendees, and the open discussion of a Full Facial Assessment. The purpose of this is not to make anyone feel uncomfortable, but to use this as a learning experience to assess areas for elective improvement. Not all areas discussed will be treated. 

• Model must consent to all treatments prior to being injected. Must also consent to the marketing photography release.

• Model agrees to having before photos/after photos taken and shared. Will have personal access via TouchMD portal.

• Model agrees to return for a 2 week, in person follow up appointment. This is necessary for documenting progress and obtaining follow up photography.

• Model is aware that they could likely bruise, swell, experience some expected injection pain, and that they will not be allowed to put makeup or chemicals on their face/ injection area for a minimum of 12 hours post procedure. This is to help reduce the risk of infection.  Keep in mind that bruises usually take two weeks to resolve, so please be advised ahead of scheduling.
